How to include images

I am struggling with how to include images in my pages, where the width is fixed (set by me), and the client can upload images, which then will be resized automatically, and a rel="lightbox" link added.
For some reason, when I upload images, no thumbnails are created. Not sure what's wrong there?

Any help for how to work with dynamic images is appreciated.

You can use this plugin for the resizing (allthough I didn't test it):
