admin not showing full admin menu

Client upgraded - I think - and now when we login as admin - it is not showing the full admin menu

I went into phpMyAdmin and admin_access is set to 1 for all three accounts

But the menu only shows

Content Extensions and my Prefs

there isn't even a way to add another user ?

What is messed up and how can I fix it

we are using CMS Made Simple 1.4.1 "Spring Garden"
