im new here and added the form

but in the fields i get this :

also in the received email i get this ?
also the two buttons is showing strange code

a other thing is i use the my company template (responsive )
when i look on a small screen the form doesn't resize
EDIT : this i have solved to use cbcontact_timbow this one is responsive

but still strange characters in my form
any idea how to solve ?
