In filling out the Extend form, I put in my theme forum ID, submitted the form, but when I click on that link it gives me a 404. It appears that it took the ID from the Extend form, and putting it before the form address that it asked for in the form. How do i correct this?

Can you just click manage my files and change it to the correct forum url.
It doesn't do it for you, you have to create a thread for your plugin in plugins and stick the link there.

Thanks everyone,... just call me a doofus, I thought you were only supposed to put in the stub. What can I say, it was my first time through this experience of uploading to extend, and I think I made every mistake in the book. ... oh well live and learn!
Thanks again!
Not only is GetSimple the application great but GetSimples forum is the best!
