GetSimple Contact Plugin additional field

Hi there,
I need your help to add a field in the GetSimple Contact plugin (Version 1.3.0 - it´s the latest one and I wonder if it´s alright to use it on recent websites because it´s kind of old?)

Do anyone of you know how to add a radio field with text (inkluding link)?
I need it to match Data security regulations which will be necessary in Europe from Mai 2018 on.

There's this relatively new form builder plugin Imforms, it might be better suited to make custom forms.
