Честно говоря у меня нет идей, я первый раз делал отправку аттача на ПХП и на тестовом сайте всё работает.
Давайте попробуем.
Включите галочку debug в настройках и в файле simple-c-functions.php
после $tmpFile = $_FILES['simple-c_fields'];
добавьте var_dump($tmpFile);
скиньте сюда, что вывело.
