Please can somone let me know how to add a contact us page with the box's ie ... EMAIL / NAME / SUBJECT / MESSAGE BOX'S .

Indeed, like yojoe said, you can do it with p01-contact.
To have the same contact form as in your image, you can use the tag...

Code:

(% contact : name! "Your Name :", phone "Your Telephone Number :", email! "Your E-Mail Address :", website "Your Current Web Address :", message! "Brief Details of your Requirements :" %)

(Create name, phone, email, website and message fields with custom titles)

...and some css for styling.

Forgot the admin password of my site. I am asking for a new password but I do not get it in email. Is it possible that somebody at GetSimple resets my account and notify me at another email account?

the safest way for you is:

backup your data
delete GS from the server
install anew (will produce a new password)
upload the data to your server
